As we know, a patch test revealing any sensitivity to hair color, is required to be
provided prior to each and every hair color appointment we deliver. Confirm this by
opening the hair color box, and by completely reading the enclosed instructions.
Past history: One of the finest colorists I know was sued by a woman who had been his
long-time client. She needed money, and decided to sue him and his salon because no
patch test was given to her prior to her appointments. The court decided in her favor, citing
that the brand's instructions clearly stated the requirement for a patch test. This cost my
friend several hundred thousand dollars......... Of course, this is one case in a million,
BUT, I thought it best to ask new clients to sign a waiver during their first hair color visit
at our salon. I've shared the copy for you here. I recommend that you consider this as a safety net you many never need to use. Of course, the choice is yours.

that anyone having a hair color application (at a salon or at home), be given a patch test prior to the date of the salon service. This test rveals any possible sensitivity. or allergy
to hair color ingredients. And, this test is to be given prior to each and. every hair color application.
By signing this HAIR COLOR PATCH TEST WAIVER, I acknowledge that I have read the
content noted here, that the salon professional caring for me here, and that (YOUR SALON NAME) salon has offered to conduct a patch test. I. (CLIENT'S NAME), direct the salon
to proceed with my hair color appointment, without conducting the patch test.
